Growing hospice, in need of a full time social worker to be part of our amazing team. Complete comprehensive psychosocial assessments for each patient/family; provide mental health diagnosis and end-of-life counseling services to patient and family based on assessed need; initiate bereavement assessment and risk for complicated grief; participate in Quality Assessment activities; actively participate with Hospice Team to provide exceptional care and support to patients, families, and Long Term Care staff. Masters Degree from an accredited School of Social Work required. LCSW preferred but will provide licensure supervision for the right candidate. One year of medical or hospice social work experience preferred. Must have reliable transportation, and a good driving record to travel to patients’ residences.
